The Eyemart Cares Foundation is a 501(c)(3) non-profit organization dedicated to providing relief and assistance to Eyemart Express Associates in need due to natural disasters, medical emergencies, financial hardship and other personal difficulties.

FAQs
What qualifying events will be eligible for financial assistance?
The Fund was established to assist associates facing extraordinary financial need based on an unforeseen hardship or items out of their control, which can include:
- Natural disaster (i.e. tornado, flood, or earthquake)
- Terroristic or military action disaster
- Flood
- Fire
- Victims of domestic violence, sexual assault and serious felonies
- Other displacement (electrical issues, carbon dioxide, etc.)
- Serious medical issue and/or medical bills
- Serious accident
- Death (loss of income, funeral support, etc.)
- Situations which occur that are created outside of the associate’s control.
What are events that would not qualify?
- Car repairs and insurance
- Legal expenses
- Property losses
- Non-essential bills, appliances or electronics (cable television, telephone, internet service, etc.)
- Debt owed to friends and family
- Wage garnishments
- Personal taxes and penalties
- Accumulated financial distress (if income is not enough to cover regular monthly bills)
Is there a maximum amount that I may receive?
Yes, each associate may receive a maximum of $5,000 within a rolling 12-month period.
Who can receive assistance from the fund?
Current associates of Eyemart Express and our family of brands are eligible to apply for and receive assistance from the Fund upon the associate’s first day of hire. That said, the incident for which the associate is requesting financial aid must have occurred while employed by Eyemart and within 90 days of applying for assistance.
How long does it take to process a grant application?
The Foundation works to maintain a rapid turnaround for all applications. The more detailed and complete your application is, the quicker the Fund will be able to respond. In most cases, decisions are made within 5–7 days of submitting your application. Please double check your application before submitting for review. You will be notified if information is missing from your application.
How will I be notified of the decision?
A member from the Eyemart Cares Committee will contact you directly. Please make sure you include a valid phone number and email address on your application form.
Are the funds received taxable?
No, these grants are not considered part of your taxable income.
Do I have to repay the grant?
No, these grants are not loans and do not have to be paid back.
Can someone else apply on behalf of an associate?
Since most of the requested information is only available to the associate in need, the associate needs to apply for assistance themself in most cases. In the exceptional cases when an associate is unable to apply themselves, an immediate family member or co-worker can apply on their behalf.
Will my information remain confidential?
Absolutely. Each application is assigned a case number in lieu of using personal information to ensure strict confidentiality with the Eyemart Cares Committee. Access to your personal information will be limited to select Leaders in the HR Team. This allows the person to contact you if additional information is needed and to coordinate payment details if an application is approved. The names of the grant recipients will be available to the Board of Directors of the Fund.
